This form is used to bring to the attention of the Federation a problem in the training and evaluation of the Quebec government's recreational scuba diving qualification.

    Did your instructor mention the regulations that apply in Quebec for divers and did he give you the necessary information to obtain a certificate of qualification, if this was your intention?

    Has your theoretical knowledge been evaluated by an exam?

    Have you learned to use the dive table to plan your dives?

    Did you initially learn and practice diving techniques in a pool or in a protected environment?

    During your pool or protected environment training, did you exercise :

    Balancing your mask and eardrum ?
    Regulator recovery and purging ?
    Breathing without mask underwater ?
    Mask removal, replacement and emptying ?
    Buoyancy control ?
    Out of air simulation and emergency ascent ?

    During your course, how many dives did you make in the natural environment, i.e. in a lake, the river, etc.?

    Did your open water certification dives take place over at least 2 days?

    Was the depth of each open water training dive between 5 meters (15 feet) minimum and 18 meters (60 feet) maximum?

    Has a mandatary personally evaluated your diving techniques during your training dives in the open water and a protected environment?

    During your training dives in the natural environment, did you practice the following techniques:

    Removing and putting back the mask underwater ?
    Compass navigation on the surface and underwater ?
    Buoyancy control ?
    Removal and insertion of weights at the surface?
    Ascent with emergency air source (e.g. Octopus)?
    Controlled emergency ascent ?
