Making a complaint
Have you witnessed an event that jeopardized scuba diving safety or a situation that was deemed unacceptable? Please let us know by filling out the appropriate form. The analysis of the information will help us to better direct our actions to reduce the risk of scuba diving accidents. Thank you for your cooperation!
Complaint and Feedback Management Process
To report a problem related to the training and evaluation of the Quebec government's recreational scuba diving qualification, please use the form entitled "Complaint Form - Certification Commission". For any other event or situation to be reported, please use the "General Complaint Form". An acknowledgement of receipt will be issued and the complaint will then be processed in a timely and confidential manner. Any complaint regarding the evaluation of an agent will be dealt with by the Commission on Accreditation within a maximum of 30 days, in accordance with the policy in effect.

In all cases, give the answer that you think is most accurate and report in your own words what you witnessed. Recounting the facts is important to understanding the events. Please do not report hearsay. If you don't know the answer to a question, don't answer it and move on to the next question.
