How to fill the forms ?

In all cases, give the answer that you think is most accurate and report in your own words what you witnessed. Recounting the facts is important to understanding the events. Please do not report hearsay. If you don't know the answer to a question, don't answer it and move on to the next question.

If you have any questions about how to complete the form, please contact the Federation at 1-866-391-8835 or 514-252-3009.
